PUZZLER Teen’s speech, vision symptoms prompt stroke alert

THE CASE
A 16-year-old Caucasian boy presents to the emergency department with sudden onset aphasia and dysarthria. He has had fever for 3 days but no headache, diarrhea, abdominal pain, or other recent illnesses. His parents report that he was completely well and at his baseline when he went to bed the night before. Upon waking up this morning, he was unable to communicate clearly and was obviously frustrated. Sick contacts include the entire family who had had flulike symptoms 2 weeks ago that have since resolved.
